New course equivalency review request form improves security, streamlines processing

The Office of the Registrar (RO) has launched a new eForm to make the process through which students request course equivalencies more secure while also streamlining backend operations.

The form is what currently enrolled undergraduate students use to request that a course from another institution that doesn’t have an equivalency listed in Transferology be evaluated to determine how it may transfer to UW–Madison. It was created by the RO’s Credit Evaluation Services team and can be found in the “Student eForms” section of the MyUW Student Center.

Moving to an eForm instead of the previous web-based Qualtrics form provides another level of assurance that the person submitting the form is a UW–Madison student. Since the student will already be logged in using their campus credentials, they won’t have to provide as much information in the form, and their data will be more secure.

Behind the scenes, the eForm will also streamline internal processing and data collection for the Credit Evaluation Services team.

What’s Not Changing

While the form itself is new, the review process is staying the same. Students are still required to provide a syllabus for the courses they would like reviewed. Courses must be from a U.S. institution and must not already have an equivalent in Transferology.

How To Use

The eForm is available now for current UW–Madison students to use. For step-by-step instructions on using the new eForm, visit our KnowledgeBase (KB).

The Qualtrics form has been shut down and is no longer available.
