Withholding Student Information under FERPA
When completing the Request to Withhold Student Information, please consider:
- Only the student may submit, revoke, or modify a request to withhold student information. This request will preclude the University of Wisconsin-Madison from releasing withheld information to anyone, including family members, even under emergency circumstances, without first having the students consent.
- This form will remain valid until revoked or modified in writing even if you are no longer enrolled. Oral revocation will not be accepted.
- The University receives many inquiries for 'directory information' from a variety of
sources including friends, parents, relatives, prospective employers, graduate schools, honor
societies, licensing agencies, government agencies and news media. In addition, on-line
directories use 'directory information,' including both the UW-Madison directory and the new
UW-System directory. Please consider very carefully the consequences of any decision by you
to remove these items from the list of 'directory information.' Should you decide to inform
the University not to release any or all of this 'directory information', any future requests
for such information from non-University persons or organizations will be refused.
For example:- The University could not release your telephone number or address to a family member wishing to notify you of a serious illness or crisis in the family.
- A prospective employer requesting confirmation of your major field of study, address or date of birth would also be denied access to such items should you withdraw them from the 'directory information' list.
- Withholding your name does not allow the University to acknowledge your attendance.
- The University will honor your request to withhold any of the ten items listed but cannot assume responsibility to contact you for subsequent permission to release them. Regardless of the effect upon you, the University assumes no liability for honoring your instructions that such information be withheld.
- If you are also an employee of the University (e.g., Teaching Assistant) and you wish to withhold information, you will also need to contact the Human Resources office as information may appear in a printed or on-line directory because of their data.
- When you request transcripts, your request will override this hold for any items of information ordinarily on the transcript, but only on that occasion and for the purpose of complying with your request.
- If you have authorized the release of your academic information to a parent or guardian, a request from them will override this hold for any items ordinarily on the grade report, transcript or other items, but only on that occasion and only for the purpose
- If your purpose in requesting this withhold is to avoid third-party "junk" mail, you might consider an alternative by requesting only a third-party hold (prevents mailing from sources outside the University). Go to My UW, click on the Student Center link and submit a third-party hold in the Personal Information section.
